Using the Page Setup
Click on the operating system that you use:
Mac OS9 | Mac OSX | Windows
You can access the Page Setup from the File menu in Photoshop, but hopefully you are using the Print with Preview window already; and therefore are one click away from the Page Setup. So go ahead and click it.
Now the main problem that follows is that the Page Setup is not a function of Photoshop per se, instead it is a function of the operating system (windows or mac).
Mac OS 9
Your window in OS 9 should look like this:

In the upper left-hand corner you'll see the printer you currently have selected. If that isn't the one you want to use, change it in the chooser and start again. In this case, I'm using our 7600_West printer.
Set the paper size that you will be using.
Note: You will need to have a paper size that is usually .25" larger, in each direction, than the image, unless you are doing borderless prints. OS 9 does allow you to make custom print sizes (by clicking on Customize...) and print without borders. OS X doest allow for custom print sizes, but will only allow you to print borderless on original Epson print sizes (formated for specific print drivers).
Set the paper source. This is important, what you choose as a source on the actual printer console matters little, this setting on your page setup means everything!!! If you leave it on Roll Paper, but have a sheet in the machine with the machine set to "sheet", it still won't allow you to print until you change the printer console to one of the roll paper settings.
To print borderless, all you need to do is select "No Margins."
The "Auto Cut" option will usually cut the paper regardless as to whether you have allowed for it on the printer console. Again, the driver reigns supreme, not the printer console.
Print Page Line works well, but you should also consider whether adding a border manually in photoshop would work better for you.
Once you have everything set right, hit OK and you'll be sent back to the Photoshop Print Preview Window. There you can see if the image is correctly orientated.
Mac OS X
The new flavor of mac operating systems are wonderful in most people's eyes, except in one major catagory: Printing is a pain!! Oh well, we'll work with it.
The page setup now looks like this:

If you want a custom print size you'll need to go under Settings: and make a custom paper size:

Once you name the new size (I've named it 20 x 26), and enter the dimensions (20 x 26), make sure you hit Save. Unlike OS 9, were all you had to do was hit OK, you must instead hit Save, and don't hit OK!
Go back to Settings: at the top of the window, and the new paper size should show up.
Windows
Page Setup in Windows doesn't do much in and of itself, at least the way you should be using it, it shouldn't. Instead, it should be used to point the system towards the more powerful Epson Driver, which has been built in Windows to do the customizing, orientating, etc. that is usaully done in the page setup.
To point it towards the printer driver, you must first have that particular printer driver selected. In the lower right-hand corner, you'll find a button labeled Printer....

If you click on that button, the following dialog box will appear, select your printer from the pull-down menu:

Once you have selected the printer you want, go into the driver settings of that printer by clicking on Properties.... Once in the driver, click on Paper to get the following options:

From here, you can set the paper size, orientation, etc. Again, this is more powerfull than the regular page setup. Once you have set everything to your liking, Hit OK until you are brought back to the regular Page Setup. It should now say "User Defined" in the Size field, if not, make it so. This means that the attributes of the page setup are defined by the Epson Driver.

Hit OK to go back to the Print Preview.
